JHUMUNC Delegate Waiver and Permission Form
The JHUMUNC Delegate Waiver and Permission Form must be filled out by each delegate attending JHUMUNC 2013 prior to the start of the conference. Delegates should hand in completed forms to their advisors so that advisors can collectively hand in the forms on behalf of their delegates upon checking in to conference.

The JHUMUNC Advisor Form must be filled out by each advisor attending JHUMUNC 2013 prior to the start of the conference. We kindly ask advisors to hand in this form along with each of their delegate's Delegate Waiver and Permission Forms upon checking in to the conference.
